Provisional Membership File

The documents required for your provisional membership file can be accessed here.

After you apply for provisional membership, you will receive an email with a list of the documents  required for your Provisional Membership file in the Office of Ministerial Services. The file must be complete in order to be scheduled for an interview with the Board of Ordained Ministry.  It is important that you save and/or print this email, so that you can reference it later.  

Please be aware that you may have submitted some of these documents, such as transcripts, previously to your district office, and you might be required to submit them again for your file with the Office of Ministerial Services.  Your cooperation in resubmitting any documents is greatly appreciated.  We also highly recommended that you keep easily accessible copies of all the documents that you submit for your Provisional Membership file.

It is important that you pay close attention to the deadlines and submission instructions for all items since there are some documents are not returned to the Office of Ministerial Services.  For instance, you will return the District Recommendation to your District Office.

If your background check is over 4 years old, you will be required to complete a new one.  If your last medical report is over 4 years old, you will be required to complete a new one.  If your last full psychological assessment is 4-5 years old, you will be required to complete a follow up assessment.  If your last full psychological assessment is over 6 years old, you will be required to complete a full reassessment.  Please note that any forms required for the psychological follow-up will need to be submitted to Ministry Development Services not the Office of Ministerial Services.
